    • Do I have "out-of-network" mental health benefits for outpatient office visits?

    • What is my out-of-network deductible, and has it been met for the year?

    • What is the "allowable amount" or reimbursement rate for the following CPT codes?

      • Individual Therapy: 90834

      • Couples/Family Therapy: 90847

      • Group Therapy: 90853

    • How do I submit my monthly "Superbill" for reimbursement? (Most companies now have an online portal or app for this).

  • Please call or text me at least 24 hours before your session; otherwise, the full fee will be charged.
